CLINICAL PHARMACY SPECIALIST – PEDIATRICS – Prestigious Boston area Teaching hospital is looking for a Clinical Pharmacist to function in a complete 100% Clinical Role in the area of Pediatrics. This will be a Monday to Friday Day shift schedule with no evenings or weekend rotation;
Participates in patient care rounds with direct family interaction, when feasible, obtains pertinent information required to determine the efficacy of drug therapy.
Provides drug intervention during rounds to the team prior to medication therapy is ordered and formulary management in this specialty area,
Evaluates compliance with guidelines or recommendations, including DUE, established by the hospital P&T Committee. Educates the appropriate practitioners to ensure accountability with compliance.
Documents all interventions provided during rounds, evaluates patient response to drug therapy and measure patient outcomes
